In connection with my consideration as a Tenant of Pleasant View Properties, I understand that consumer reports or investigative consumer reports will be requested on me, which may include searches relating to the following: address history, criminal records, employment history, education, civil court filings, driving records, and registered sex offender status. Reports may also include but not be limited to: my work habits, salary history, performance, education, experience, reasons for termination of employment, and any history of criminal, dishonest, or violent behavior. Further, I understand that requests for information may be made at various private and government agencies that maintain records concerning my past activities.

I hereby authorize Pleasant View Properties, Matthew E. Brown and/or Melanie A. Brown, or any of their authorized agents or attorneys, to obtain the information referred to above. I authorize, without reservation, any individual, corporation or other private or public entity to furnish to Pleasant View Properties, Matthew E. Brown and/or Melanie A. Brown, or any of their authorized agents or attorneys, the information referred to above. This authorization extends to current and former landlords, employers and bank representatives As allowed by law, I release Pleasant View Properties, Matthew E. Brown and/or Melanie A. Brown, and their agents and attorneys, and all individuals, corporations, or other private or public entities providing information, from any liability for damages that may result to me as a result of furnishing or attempting to furnish such information. I have received and reviewed a copy of “A Summary of Your Rights Under the Fair Credit Reporting Act.” This Consent, Authorization and Release shall remain in effect during the entire process of my being considered as a tenant of Pleasant View Properties. This investigation is for tenant screening purposes only and is strictly confidential.
